This is a two sided piece with a double taper on the girdle. 30x25x7 mm Low domes on both sides. (sorry about the dust)

My understanding is this material originates at Mt Tahoma, near Mt Rainier, Washington State.

Nice. I don't recall seeing any cut that direction to show the cloudy plume tip pattern. Agree, Tahoma is from Washington (Tahoma being the ancient name for Mt. Rainier).
